HOW LONG THE NOSE JOB IS?
We also hear stories about other cosmetic plastic surgeons, and how they function, from potential nose job patients. Most recently, we have learned about a first nose job performed elsewhere that takes three to five hours from many patients undergoing revision rhinoplasties (the operation to correct a failed first rhinoplasty). Most of those long, long nose job procedures, even more dismaying, created only a frustrated patient who still had an unattractive nose. We find that dismaying in the extreme.
We find that nose operations consume so much time when we ask those patients some questions because the first surgeon spent more time than normal grafting and transplanting nose tissue. There are certain cases such as accidents or previous nose surgery that have ruined some of the nose tissue of the patient. But the unusually long-time issues remain. We are not sure why it takes so long to perform these nose surgeries. Our fundamental reaction is that some surgeons make the procedure more complicated and involved than it should be.
In most first rhinoplasties, here’s what happens: if the surgeon needs to perform transplantation or grafting inside the nose, inside the nose, adequate tissue is found. Doing so makes the process of extracting nose tissue very economical and efficient since it is accessible in the same region in which the surgeon is operating. We also use nasal septum cartilage and/or bone, the paper-thin section of bone and cartilage that splits your nose into two channels. We take it out in cases of a deviated septum to strengthen the airway of the patient and to correct circumstances that may lead to sinusitis or other complications.
Cartilage for the nose in rare cases may come from the ears of patients. But now, even though certain tissues will bend and curl, I’ve heard of initial rhinoplasties in which rib cartilage or even rib bone is used within the nose. But the point is: the use of ear or rib cartilage allows for a lengthy, complicated and involved operation since it is important to open another surgical site. Nonetheless, we conduct almost all of our nose operations from inside the nose and use secret incisions. We close with absorbable stitches. But the other procedure, open rhinoplasty, allows the surgeon to expose the inside of the nose by peeling back the tip of the nose. To work on the engine, others equate it to opening a car hood. The “Open” technique also requires an external incision on the nose.
